Medical insurance in Europe
 
Please can someone tell me what form(s) I need to extend my National Health cover when in Europe.

It used to be called form E111 (sounds like poison, doesn't it?). Supposedly has been or will be replaced by a plastic card. Contact your health insurance/public health system for this.
