It might mean two things
Iranian vehicles need international plates to leave Iran - plates with Latin rather than Arabic script, so it might just mean Latin script licence plates.
or,
Years ago, foreign vehicles were given Iranian transit plates when entering Iran, but I have not heard of this happening to tourists for many years now.
Either way, nothing to worry about.

The info came from a post from meh Mehmet Zeki Aviar 23 October 13
I emailed an Iranian travel agent and they claim you need to get international plates to enter Iran
I've never heard of this and will check with RACV (local AA)
Just another obstacle to overcome I suppose but I hope it is nonsense!

Int'l license plate
I think there is a confusion!
Iranians need such an int'l license plate with latin letters and "arabic" numbers readable abroad in case they want to leave Iran with a vehicle registered in Iran as their Iranian plates are in Farsi...
Foreigners from western countries coming to Iran usually already have plates in this shape anyhow.
So, no worries!!

@branners
Lets make it clear.
According to the int. agreements and laws for automobile clubs, means carnet and country sticker for private vehicle makes your number plate international!!!! Thats a very old saying but still used in black books..
Thats why the sticker with your home country code letters(black letters on white background and TR for Turkey and GB for Great Britain, etc.) on it is given by local automobile club together with the carnet..
Thats the email I got from a friendly tourism agent in İran with the link to the webpage to apply for visa code number without making any front payment.
If they dont do it as they mentioned, means they dont keep their promises.
@FLH80.
For Turkey its not a must to have number plates in latin alphabet..Currrently many cars and bikes in İstanbul with farsi or arabic number plates..You just translate it at the gate while buying traffic insurance policy..
Hope helps..

I think Mehmet made it clear; it's a language/ translation confusion.
To call it an "International Transit Plate", per OP, isn't quite right.
That terminology might suggest a different/ separate plate from what
is issued and used in your--most folks'--home country.
Maybe more accurate to say: be sure your national plate has a country
ID sticker added, if the country abbreviation isn't already part of/ embossed on the original plate. Most newer plates already include the country code.
But not everywhere. For example (to my knowledge), none of the US states or Canadian provinces issue their motorcycle license plates with a separate 3-letter abbreviation for the country, USA or CDN. Those plates only spell out the name of state or province. It's also not easy to find a 3-letter abbreviation sticker that fits on a motorcycle plate; and CAA, the CdP-issuing auto club, does not send out stickers for motorcycle Carnets.
If you can't find an "official" sticker, make one:
