I sent my bike from Brisbane to Bangkok through Qantas. I didn't need to create it. I just turned up a few days before i wanted it to arrived in Bangkok.
You need the following
customs clearance number - you get from the customs office in the city (takes about 2 days)
Dangerous certificate which you get a couple of hours before you drop it off at the airport. You need an address at your destination. I used a random hotel address.
Qantas charged on the weight of the bike rather than the volume - which saved me alot of money.
